Crystal Storing Histiocytocis
In this rare condition there is massive accumulation of
crystallized immunoglobulins in nodular collections of
histiocytes77,78 (Fig. 4E). It is usually associated with
plaa cell dyscrasias, monoclonal gammopathies, and
other lymphoid neoplas. In these situations it may
manifest as a systemic process (crystal storing histiocytosis),
although it may also present as a mass lesion in the lung or
other organs including the kidney, liver, and bone marrow
among others. An association with collagen vascular
disease (rheumatoid arthritis) has also been described.
Immunohistochemically, the histiocytes have been shown
to be positive for CD68, CD43, and lysosome, and weakly
positive for k and l light chains, and IgM. S-100, B-cell and
T-cell markers (CD20 and CD3), and cytokeratin stains are
negative. In the context of an underlying lymphoproliferative
disorder, the prognosis is thought to be poor, although in
cases with an isolated pulmonary crystal storing histiocytoma,
it may behave like a reactive entity with good prognosis.77 |
